SCUBA diving is a physical activity, and can be strenuous at
times. Therefore, in order
to participate, a person must maintain
a reasonable level of health and fitness.
Additionally, there are certain medical issues which must be considered. This is the basis
for the PADI Medical Statement. We have provided the form here, so that you may read it,
print it out, and if necessary, have your physician sign it, before you begin
your training at Ocean Divers..
As stated in the form, diving is a very safe activity,
"When performed
correctly, applying correct techniques...".
Personal safety requires
adherence to basic practices of safe diving. Failure to do so can be dangerous.

PADI
MEDICAL STATEMENT
A signed copy of this form
is required for each student, for any PADI SCUBA training course. If you
have any of the pre-existing
conditions, you will need to get the approval, and signature, of your
physician, to engage in SCUBA diving/training. Since some doctors are
not familiar with the special needs/risks of SCUBA diving, the form
includes basic guidelines and other information for the examining
physician.
